يجب أن يشتمل التبليغ على البيانات الآتية :
- يوم الولادة وتاريخها
- نوع الطفل ( ذكر أو أنثى ) واسمه ولقبه .
- اسم الوالدين ولقبهما وجنسيتهما وديانتهما ومحل إقامتهما ومهنتهما .
- محل قيدهما إذا كان معلوما للمبلغ .
- أى بيانات أخرى يضيفها وزير الداخلية بقرار منه بالاتفاق مع وزير
الصحة .
