يجب على الموظف المختص بمكاتب الصحة تسجيل التبليغات المستوفاة لجميع البيانات المؤكدة لصحة الواقعة وبياناتها بدفتر المواليد الصحي فور تلقي التبليغ وتسليم المبلغ شهادة التحصين ضد الأمراض المنصوص عليها في المادة (4) من هذا القانون.
