تقدم البطاقة الصحية عند كل فحص طبى للطفل بالوحدات الصحية
أو مراكز رعاية الأمومة والطفولة أو غيرها من الجهات الطبية المختصة .
ويثبت بها الطبيب المختص الحالة الصحية للطفل ، كما يسجل بها تطعيم الطفل
أو تحصينه وتاريخ إجراء التطعيم أو التحصين .
