Article 64 of the Physiotherapy Syndicate Law

يجب على كل من يحمل أحد المؤهلات المنصوص عليها فى المادة الثانية من القانون رقم 3 لسنة 1985 فى شأن تنظيم مزاولة مهنة العلاج الطبيعى أن يقدم طلبا إلى المجلس المؤقت، وذلك خلال ستة أشهر من تاريخ نشر هذا القانون فى الجريدة الرسمية لإدراج اسمه فى جداول النقابة بعد سداد الرسوم الخاصة بذلك.
ويتضمن الطلب لقبه وجنسيته ومحل إقامته ومهنته ومؤهله العالى وتاريخ حصوله عليه وتاريخ مزاولة المهنة ومقر هذه المزاولة.
